Our data, listings or other constituent elements may not be downloaded, republished, resold or duplicated, in whole or in part, for any purposes including the compilation of mailing lists. Use of our site's materials and database for any purposes other than personal research is strictly prohibited. By using our service you agree to the terms and conditions set below.

PHYSICIAN REFERRAL SERVICES:
(1) Your submission to us constitutes a request to us for a physician referral. This referral will be forwarded to the physician(s) of our choice. INTERNET MEDICAL GROUP reserves the right to determine which submissions for referral will be considered. (2) When using our service, you agree to provide true, accurate, current and complete information about yourself as prompted by our submission form. (3) No advice or information obtained through the use of our service shall create any warranty. You use our Referral Service at your own risk. INTERNET MEDICAL GROUP will not be held liable for any injuries or medical malpractice(s) and or claims for remuneration associated with any form of damage, foreseen or unforseen incurred by your visit to, or contact with via any medium, any and all physicians or medical services contacted as a result of your use of our Referral Service.

ONLINE FINANCING
Applicant agrees that applicant is at least 18 years of age and warrants that all information submitted is true. Applicant authorizes Internet Medical Group to submit financing application to the financing company of Internet Medical Group's choice. Applicant understands that Internet Medical Group is not a financing company and has no control over whether or not an application is accepted or not. Applicant understands that all applications are final. Because applications are forwarded to a third party (finance company) submitted applications may not and cannot be cancelled. Please note that the USPS PATIENT FINANCING APPLICATION FORM is not administered by Internet Medical Group. By using this service you allow Internet Medical Group to transmit your financial and personal information to a third party of our choice in order to facilitate financing your surgery. By using this service you also agree that Internet Medical Group will assume no responsibility and will not be held liable for the misuse of information by any parties to which this information is transmitted. By using this service, applicant understands that all information submitted may be shared with a third party and that some of the information about applicant or applicant accounts may be shared among Internet Medical Group and its related affiliate companies and/or partners for marketing and/or administrative purposes.
